The Position: Patient Care Nurse Navigator

Employment Status: part-time

Hours per two-week pay-period: 28

Essential Job Functions:

  • Educate patients and families about plan of care
  • Collaborate with healthcare team to ensure coordinated patient care
  • Advocate for patients and families; connecting with available resources

Minimum Qualifications:

  • Graduate from accredited school of nursing
  • 5 years of nursing experience preferred

Certifications and Licenses:

  • Current unencumbered Montana RN License

Position Summary:

Sidney Health Center is seeking a Patient Care Nurse Navigator to join our I CARE team in the Medical Oncology Clinic setting. Essential duties of the position include excellent oral and written communication, keen attention to detail, ability to work independently and collaboratively with interdisciplinary teams.
